Notes:
Age extracted from an application for a delayed birth certificate, request dated June 18th, 1982, application sworn July 27th, 1982 and notarized October 4th, 1983. No farther attempt was made at getting the certificate.
Phillip lists his parents as John Moser, with out the Sr. attached and Anna Marie Lebenberger.
At the time of application his address was 565 E. Gloucester, Gladstone, Oregon.
Proof of age documents had included a retirement application record from Crown Zellerbach from September 23, 1952; an army discharge record; a family bible record; birth record of his son, Phillip, who was born May 31, 1944 in Multnomah Co., Oregon; an affidavit from his sister, Bertha; and the original baptismal certificate from Trinity Congregation signed by C. Ed Mix, Pastor, dated October 02, 1920.
I have copies of the retirement record showing application for employment was October 31, 1941, the baptismal record as translated from German by P. L. Moser, and the application for birth certificate. I received these from the North Dakota Department of health with a letter November 8th, 1999.
Phillip was Baptized in the American Evangelical Lutheran Church, Pettibone, North Dakota, October 02, 1920. The Pastor was C. Ed Mix and the Godparents are Jacob Flemmer and Mrs Jacob Martin.
The Baptismal record was written in German and translated by P. L. Moser.
